The city of Rhode Island has an application for sidewalk repair on their web site; I believe you can fax it in.  I applied for repair about 7 years ago, and then again a few months ago.  I do not know what the timeframe is, and am similarly interested in hearing about other folks' experiences getting their sidewalks fixed.

Ten years ago, after purchasing my house, an application was filed with the
city (going through the proper steps) to repair the curb stone. To this day,
after many calls to various City Hall Departments, NOTHING has been done;
thus my vehicles have sustained damages.

Would someone like to shed some light on this issue as was done with the
fountain, i.e. preventative medicine for the vehicles as well as being able
to safely walk, and enjoy the neighborhood.
